Telehealth Nursing & Chronic Care Management
In 2015, Medicare began reimbursing healthcare providers for telehealth calls in-between doctor’s visits, to improve health outcomes for patients who have multiple long-term chronic illnesses. These calls provide patients an opportunity to speak with healthcare professionals on a frequent basis, which increases the likelihood that treatment plans are adhered to and a patient’s health concerns are addressed sooner rather than at their next in-person visit.
As healthcare providers shift towards outcomes-driven business models, many want to provide these telehealth check-ins, but struggle with the operational overhead of scheduling calls, managing staffing, qualifying patients, filing for reimbursements, and so on. Understanding this challenge, Raphael Anstey and team developed a unique software platform that powers CircleLink, enabling them to provide outsourced chronic care management using a network of registered nurses across the country. The platform allows CircleLink’s nurses to focus on the value-added task of speaking with patients, and abstracts the operational overhead which providers would normally struggle with.
We think CircleLink will continue to benefit from broader trends in remote-work and telehealth, and were particularly impressed by Raph’s foresight in understanding the efficiency that a software-first approach can bring -- which now serves as a competitive barrier. We’re also excited by Raph’s vision for the future of CircleLink in leveraging connected health sensors to further improve patient outcomes.
Riverside has a long history in investing in the healthcare sector, which we look forwarding to bringing to CircleLink. We’d also highlight this as an excellent use-case for RAC’s unique form of growth capital, which limits dilution and maximizes optionality for fast-growing companies like CircleLink.
